Am I the only one who has never seen their long form birth certificate?
I'm 35 and have never laid eyes on the long form. I had the short form to get my passport and that's it. I guess I'm not American yet? lol

2. I didn't even have a birth certificate until I was five.
I was born on a farm with no doctor even present. When it was time for me to enter school, I needed a birth certificate. My folks wrote to the town nearest where I was born, and they issued me one. It is a very unofficial looking document.

3. I know a long form exists, somewhere,
But I've never had need for it. I have a card with the birth data on it, and a card with my date of marriage on it, and that's it.

4. I actually have a certified copy of my long form in my hand.
It has all the names, dates, and pertinent information under a heading of "Certificate Of Live Birth".

I was born in '55, and got this copy in 1971, and have had it ever since.

8. Didn't Even Know There *Was* Such a Thing
My birth certificate has no middle name. Has Newark, NJ, the date, and not much else. Not even sure if that document is even in the "Important Papers" file.

9. Hell I didn't even get a birth certificate until I had to sign for the draft
Born at home as all the rest of my brothers and sisters were. By the time I got my birth certificate the doctor who delivered me was already dead and gone. I don't know who signed it? maybe my dad. If I knew where it was I'd go have a look see but I've no idea as to where it may be.

10. No.
I do not have an original, official birth certificate. One was not made when I was born. Later in life, one was made.

11. Nope. Only have the short form
and that has served me well to get any legal documentation I ever needed, including a passport.

12. I have both. I couldn't locate my old one, so I ordered a new
one from the county in Arizona where I was born. Before it got here I found the old one with a nifty embossed seal. It's worn and tattered. I took both to the Social Security office to sign up for SS. The lady there didn't like the old one, but thought the nice new printout was great. Weird.

14. I have 2 copies of my "long form" - my husband has a 'short form,' the only one I'd ever seen
Oddly enough, my brother, who wnas born in the same hospital as my husband but three years later, has the old 'long form.' Why? Due to when we each needed ours and which ones we requested.

Who the hell knows anymore? I just get the copies the county gives me and nobody blinks; passport, jobs, licenses, whatever.
