Spent seven of the last ten years nursing my ailing parents. Dad has senile dementia and ended up with a major stroke and my mother had congestive heart failure and was for about, five years until her death, on 24/7 oxygen.

45's trip to the hospital reminded me of how my mother's illness progressed. She began by using oxygen a couple of hours a day and then she had a series of panic attacks, during which she could not catch her breath. These resulted in brief trips to the ER.
I believe this is what happened to 45 when he made that unexpected trip to the hospital.

He also has definite signs of senile dementia, and sundowner syndrome. Perhaps he is also suffering from TIA's that are closing down some of his neural pathways.

It has been said that he is spending almost no time in the office and most of his time "Working" in the residence. My guess this is to hide his increasing dependance on oxygen. I seriously doubt they will be able to prop him in public much longer.

Just MHO based on my journey through the medical books in caregiving.

This is what Nancy is referring to when she says she is praying for the president. She probably knows a lot more than we do about his health. She has also questioned why his family has not intervened.

His actual condition is probably a national security problem on the deep DL at this point.

The key to his winning is all the rallies he attended (well, yes that and a bunch of other stuff). But if as the OP says, he is deteriorating, then I'd think those rallies should be a thing of the past. In fact, over the past few months, I've read that the crap he spews at those rallies is making less and less sense. Mostly a series of brainfarts.......

... but we now know that the Russian interference (with the help of Facebook and Cambridge Analytica) was key to suppressing the votes in a swing state. THIS IS HOW TRUMP WON!

Using what can be called psyops (a military term) with the help of Cambridge Analytica data the Trump team was able to target persuadable (aka swing voter) in each district [via Facebook and other social media] to do one of two things vote for Trump or stay home.

...rallies of her own. And those selfies with the public may, through Facebook and Instagram postings, multiply — and with more profound voter turnout than a cheesy, lie-infused psyops ad. If only King Zuckerberg would kill the fake ads... then truth-based campaigns would have a fighting chance.

We should put 90% of our elections battle towards getting colleges to divest in AM Radio and Zuckerberg to go to AA (rich Assholes Anonymous)...

It's lining and will continue to help line their avaricious pockets with taxpayer-funded boondoggle. The *richer* he gets off receipts from Secret Service contingents and visiting foreigners staying at his hotels, resorts and golf clubs, along with jinked up stock market manipulation(s), the richer his estate will be when he shuffles off the old mortal coil (which I hope is really soon regardless of the cause).

Also, the Pentagon is saying they will not go along with bombing cultural sights in Iran. WHY did they go along with this assassination? Weren't they told about it? From all indications, Pence and Pompeo are religious nuts hankering for Armagedon to take them to Heaven. We do not need THEM making decisions for the entire country.

He has been on something psychotropic/neuroleptic for a long time but probably tries to go off of it, because he doesn't like how it chills him out. The medication doesn't let him "be himself". Then in times he needs to be more 'in control' he decides, or someone convinces him, to take it and the tardive dyskinesia reappears or becomes more heightened in affect. I always still see lessened signs of it even when he is on his rants, functioning at 'his baseline'.

That set lee and paul off. They get those every week. It's probably just an eye roll event, since they can pick up a phone and get answers from th wh directly. These briefings are for the minority.....

The senators know, and it is scaring them senseless its about to become public. They know they will be held accountable. So now they are shocked the wh is incompetant and the potus is a senile weirdo.

Pretty soon, trump will be that old guy shooting up an apartment management office for all to see.

My husband has it. I would expect him to know what to look for. The breathing - the forced heavy breaths trump takes.

The flat, wooden delivery is what stands out for me. Quite a contrast with his spirited speechifying at his rallies. So there’s an inherent conflict in evidence when he’s probably got aides leaning on him to play it straight ‘n’ somber when he likes the raucous rally mode a lot better. So he’s probably feeling uncomfortable having to stay controlled and walk a tightrope like this. It’s not him, and it’s not his comfort zone.

Remember, this is the guy who lives for the show. The pizazz. The razza-ma-tazz. The lights! The smoke machine! The staging and posturing and the prancing and the puffery. He doesn’t like having to stand still and behave, like the spoiled unruly five-year-old he really is, deep-down.

And yet, our pResident Five-year-old has all these underlings he supposedly outranks telling him he has to be a good boy and mind his manners during this solemn announcement when he’d rather pound his chest and preen and prance, and break both his arms patting himself on the back. He hates being told what to do. Doesn’t think he should have to.

That’s been his life-long behavior pattern: “rules don’t apply to ME!” Unfortunately, we’re stuck with that behavior pattern. It’s baked in. It was baked in LONG ago. Our so-called pResident, the adult-size five-year-old, is far beyond much of a corrective remedy. The time to correct that bad, spoiled-brat behavior was when he was five years old, if not even earlier.

“Will not be toleride”? That’s not a misprint. I made a point to write it down phonetically after I heard it. Ran it back and replayed it just to make sure I got the quote right.

And then he had a second one. “These historic accomplima shades - are strategic priorities.”

“...accomplima shades”? WTF? What the heck are “accomplima shades”? And what kind of verb is “toleride”?

Seriously? And what gets me, also, is knowing how most people misspeak and recover. I’ve seen and heard this a million times on the air, from others, and even myself. You misspeak and then immediately correct yourself. In the same breath. In the same thought. “... will not be toleride - or rather - tolerated...” Or “toleride - excuse me - tolerated.” It’s done ALL THE TIME! Somebody garbles or misspeaks, and then corrects themselves immediately.

That’s because they’re basically paying attention. They’re actually In The Moment. And fully aware of what they’re saying. If they’re reading off a TelePrompTer then it’s right in front of them, written out. And if so, they probably rehearsed it at least once or maybe twice if they had time. And he certainly did. This garble-job happened the following morning. He had all evening and up-at-3am the next morning tweeting time to do a little bit of rehearsal on it, especially with a speech this serious, on an assassination of an Iranian heavyweight.

And he’s a TV reality show personality, for Pete’s sake! You think he didn’t do rehearsals? You think he didn’t rehearse that convention appearance with the special lighting and the big fog machines on that stage on nomination night? He’s “Mr. Showbiz” forcryingoutloud!

I think the problem with yesterday’s speech was part unruly annoyance that he had to behave himself and play this straight, and part medical. Physical discomfort of some kind. Maybe he was on meds for some ailment or condition or shortness of breath or congestive heart failure or some such thing.

I’m no medical expert but I have had a lot of experience around cameras and mics and reading copy live. This stuff just stuck out, BIGTIME. I could not help but take note.
