2. Demand of whom? Obviously Trump's government isn't going to do it

and the states can't get the materials to do it. The chemicals needed for the tests are in increasingly short supply and Trump is making the states bid against each other for everything. Demands are falling on Trump's deaf ears and on helpless governors.

Italy and South Korea, have a testing rate of less than 19 per 1000 population. US is about half that.

14. Can't tell you what the sample size should be, but

a large, random sample could answer most questions. Would be a great service for a group like 23andme might attempt as they have enough genetic info to answer a ton of questions. We are now only looking at those that show symptoms.
