I think this was his idea since no one tells him what to do. This was his decision to pretend he didn't have it and infect others, hoping he'd remain asymptomatic.

But as the day goes on, it made sense because he was in a bad condition and needed to go to the hospital. If he didn't show any symptoms or wasn't having so much trouble, I have no doubt they would have hidden it.
