people isolated or fled from anyone who had Bubonic Plague. Whole households of nobility and aristocrats fled from the cities to country estates to escape infection. The poorer classes were left to fend for themselves, but they devised means to isolate the sick.

It's not as if nobody ever figured out that if one person gets a disease, people near them will get it too. They didn't know about microbes, but they knew that breathing the same air or touching the same objects or a sick person could spread a disease.
