If you lost your COVID-19 vaccine card, you may not be sure how to retrieve proof that you got the shot. Proof is being required across the country with the recent federal vaccine mandates, requiring your employer may need to see proof of vaccination before you return to the office. And nowadays, your vaccine record is needed to get into restaurants, Broadway theatres and gyms in New York City, San Francisco and other cities.
But what happens if you lose it? The Centers for Disease Control and Prevention doesn't keep a record of your vaccination status so it's up to you to keep ahold of your card. But if you happen to misplace it, most vaccine providers do keep a record. There are several ways to get a copy and store your COVID-19 vaccination card on your phone. But there's one big red flag to avoid, no matter what (we'll explain below).

What to do if you got vaccinated at a pharmacy
If you got your vaccination at a pharmacy, you're in luck. Your pharmacy most likely has a record of your vaccination. For example, at Walgreens, you can take your driver's license or ID to the pharmacy to get your vaccine record.
And you don't even need to leave your home if you use CVS. You can get proof of vaccination quickly on its website or the CVS Health app. You can also visit the pharmacy to get a new card. We suggest calling your pharmacy to ask what its protocol is if you're unsure or see if there's an online portal to request your information on its website.

(11,686 posts)purse. Every medical office I have been to has a copy. I didn't even ask if the wanted it. I had it out when I walked up to the desk. I guard mine. I got my vaccines at a medical clinic so I feel safe. However, I think it was loaded onto My Chart so I have it on digital also. Again, I am very protective of it. Thinking about getting it laminated.

I apologozed to pharmacist, explaining.
She said "no problem at all!"
She printed stickers for me & made a new vax card. 😊👍
