That's a picture of some version of the Air Force salute, in which you hit yourself in the head, and stagger backward, almost falling over.

My standard blessing is just me standing there and making a cross sign with my hand, and directing ambient blessings at you. It doesn't hurt at all, and there's no danger involved, unless you're very drunk, in which case, you probably won't hurt yourself even if you do fall over.
