He is senile, and getting more so every single day. With senile dementia, the patient simply can't differentiate between reality and imagination. They both seem equally real, except that the conjured experience, like a dream, often has some clear elements, but also some very foggy elements. Trump really believes that he talked with one of the Presidents. And when he was pressed to name names, he panicked because that was one of the foggy elements. He stammered for a bit and then went with the "protect his reputation" bit.
This was probably terrifying to him because, after challenged, he probably started to realize that it might not have really happened. In caring for cases of senile dementia, professionals advise caregivers not to challenge the stories because that usually leads to great anxiety. It is better to just let it go because the patient will wander off mentally to something else in a few seconds.
That's where we are. We now have 340,000,000 caregivers who are expected to baby this senile dementia patient.
