As I understand it, nearly 70% of nursing care residents are on Medicaid. The majority of the facilities are for-profit institutions, and keeping good and competent staff is very hard, as pay is low, and the work is hard.

It's hard to imagine how these facilities can be squeezed much further and give decent care to the residents. While staff is the major cost, food and medicines and other routine items of daily living must be purchased with available funds.
