1) instead of the signal going to your physician's cell phone, your car is equipped with a device that can only be activated and started after you've taken your "meds" (whatever they are).

when digital pills have taken over the drug industry entrepreneurs fake the patches that makes the call and steal the drugs replacing them with placebos. Millions die, get progressively sicker and spread disease amongst the population as they take fake pills and their patches make the calls faithfully telling that the pills are taken as directed at much higher rates than ever.
