Amy, real name Amanda Patricia Bossingham, submitted an application for a line of credit with M & I Bank using the social security number of another person on August 16, 2001, and the bank approved a credit line in the amount of $15,000.

The court documents obtained by Radar go on to state that Amy was convicted of one count of Misuse of a Social Security Number and in 2004 was sentenced to 14 months in prison followed by 36 months supervised release, and ordered to pay $36,294.95 in restitution.

It’s also noted that after Amy was sentenced in 2004, she was placed on four years probation by the Fourth Judicial District for Colorado Springs “for the similar offenses of Theft and Forgery.”

Amy, now 40-years-old, submitted a motion to extend her self-surrender date and modify her sentence in June 2008 — with her attorney making excuses for why the restaurant owner committed the crime.
