It's actually a security concern and a medical one that flight-crew be able to access the lavatories even when the door is locked. On those planes where there is not a release latch, the door has an override key...but that's less common as time goes on, people misplace the keys or have to go grab the key in the midst of an emergency when every second matters.

It's a good place to barricade yourself as a problem passenger. There's also the not-small issue that altitude makes people slightly more prone to cardiac arrest or myocardial infarction...better than 1 in 3 will have that heart attack while pooping. Or they feel sick in their seat and go to the restroom to throw water on their face or walk it off or get a paper cup of water...and lose consciousness in the can.
