some slippery bits fall to the floor that he does not see, until he steps on them, causing him to slip and fall, with his head hitting the toilet bowl causing him to lose consciousness, wherein he falls face-first into a soiled adult diaper, briefly regaining consciousness to realize what just happened and the humiliation he'll face if discovered with his own excrement on his face, just moments before his heart stops, and indeed his fears come true and he's discovered face down in his own diaper? Is that how it happens? Maybe?
