The way most people who did this got caught was through the signature match - most high schoolers didn't understand that a signature match would be applied, especially if your score went up by more than expected for a second test, or was exceptionally high in the first place. I know three people who paid to have the SAT taken for them, and they all were disqualified for signature match - this was 1990-1992.
I'd expect it was even easier and more common in the mid-to-late 1960s, especially with the draft and Vietnam. Understand that having somebody take SATs for you is another form of draft evasion - if he could not have gotten into Fordham (not a particularly high bar), he might have been out everywhere, and there goes his draft exemption.
