4. They had one at the first Olde Spagetti Factory in Seattle back in 1972/73.

Not as large as the one shown, but it did have a "fiddle", a whistle of some sort, another wind instrument (or two - I don't remember), and a full percussion set along with the player piano portion. Drop a quarter in the slot, and it played for a good 3 - 5 minutes. The first restaurant isn't there any more, wonder if it's still in one of the others.
