That was hinted at in the big conversation between Tyrion and Varys.
I did not think about Dany may have taken in some poison and that caused her madness.
Interesting possibility posted here.
Maybe she couldn't hear the bells?

I think Tyrion will have terrible guilt because Varys was right.

What will Tyrion do?
