Outsourced Bin Laden capture to Afghan warlords & US troops diverted from Afghanistan to participate in Iraq invasion. As a consequence the probability of Bin Laden capture is reduced.

There never was enough troop strength in Afghanistan & Iraq to suppress disorder and allow the Afghan & Iraqi people's lives to return to normal, breeding resentment & freedom fighters.

In general, just pisspoor leadership at the top. As a CiC, Bush is just a pathetic laughingstock. With a decent CiC, BIN LADEN SHOULD HAVE BEEN CAPTURED AND AN IRAQ INVASION SHOULD NOT HAVE BECOME A QUAGMIRE.
