6. People collapse from heat exhaustion at rallies all the time, not just Trump's

I remember several times in 2008 and 2016 where people passed out at Democratic campaign rallies. One Clinton rally in particular (can't remember if it was in 2008 or 2016) where she stopped speaking and asked if someone passed out, then directed everyone to move away and for the person to be given water and medical assistance - she handled it really well. That was one of several instances of someone passing out at a summer rally due to heat exhaustion. It's kind of inevitable in many places, because the places large enough to hold big crowds often lack shade, you have to get there hours before the candidate due to security protocols and candidates always running behind, etc. If the Trump campaign didn't have personnel on-site to provide medical assistance, that's certainly a problem, but people pass out at campaign rallies for other candidates too.

That said, any investigation into the Secret Service and local law enforcement's failure to protect Trump on Saturday should definitely include looking at whether his campaign refused to cooperate or rejected any suggestions from the Secret Service or local police.
