94. There are a number of issues here
1) The assumption that an orgasm "on the spot" is the goal. This is not necessarily the case. The act may be so rare and such a strong force that the "giver" uses it as fantasy fodder later. Let's not be "orgasmo-centric" when it comes to sexuality.
2) However, if we DO want to be orgasmo-centric about it, the very notion is not too difficult: "Look, Ma! No hands..." If you follow...
